By accessing or using services, you hearby consent to the storage, collection, use, and sharing of information as outlined in this document. If you use these services on behalf of another entity or person, you represent that you are authorized to accept this privacy policy on his/her/their behalf.

1. Overview

Haven, Inc. ("us", "we", "Haven", or "MyHavenHealth") is committed to respecting the rights and privacy of our users/visitors of myhavenhealth.com (the "platform", "site", "website" or "app"), including related services and applications. This privacy policy covers our practices as related to the collection, storage and use of information and data. Our company is quite new, so we intended for these agreements to be updated regularly, with the effective date clearly visible at the top of this page.

This privacy policy does not apply to any external digital or web services that are accessed through this website, whether third-party or business parner. Each of these entities have their own policies/practices that might differ from ours.

Additionally, the use of our product and services is subject to the full Terms of Use, available on our website.

2. What Information Is Collected

Personal Information:

  1. Contact data (email addresses, home addresses, phone numbers, etc.)
  2. Location (e.g. "share current location" on your phone)
  3. Insurance coverage data (payor/carrier, plan, group ID, member ID, effective dates)
  4. Employment data (name, etc.)
  5. Limited medical data (e.g. provider/doctor/dentist "favorited" to your profile, whether or not you have been to them before, simple user surveys (e.g. do you have a doctor that you go to?))
  6. Public content (e.g. user comments, ratings, profile pictures, names, etc.)
  7. Other information used to identify the account (e.g. passwords, user comments, etc.)

All information is provided by you voluntarily. Services will not be able to function without these.

In addition, other information may automatically captured when users use our services, including but not limited to: IP addresses, device used, access date, and other web-browsing / app statistics.

If the product or services is accessed through an app store or content delivery platform network, access/download information is collected as available by the distributor.

3. How Information Is Collected

Information is collected through free text input, button presses, check boxes, and switches as provided by the user while using the app. This includes (but is not limited) user profile forms, search forms, and information forms.

Traffic information and other app statistics are tracked by user requests to the server which uses logging. A similar approach is used for third-party app stores and other content delivery platforms that provide tracking.

We also reserve the right to aggregate application from the data with public third-party information into our CRM systems (for example, to send targeted email campaigns).

4. How Information Is Used

Information is first and foremost used so that the platform can operate. This means providing a customized user experience that provides access to health care based on your personal information, as well as public information.

For example, personal information is used to create a user profile, allow private login/access, providing customized search results, and to send communications back and forth.

As our platform grows, we hope to be able to be able to provide assistance in comparing, selecting, and choosing insurance. This may involve using your personal information to provide targeted advertising / customization.

We also hope to help enroll you in new insurance. This requires submitting personal information (with your consent) to the involved third parties (e.g. a broker, insurance company, etc.).

It is important to note that our company is still quite young and that our uses of information can change quite rapidly. We are thoughtful about the use of data as we grow and evolve. Please review this section periodically (and as updates/ammendments are published)

5. How information is shared

Information is not currently shared; however, we reserve the right to share and use information both internally and externally (i.e. with partners) to better serve advertising, productions, promotions, and other forms of user engagement. Some of these practices require aggregating with other public data sources, both free (e.g. google searches, facebook searches, etc.) and commercial (e.g. paid database)

Some elements of the platform, such as first name, last name, profile picture, user reviews and ratings, become public information. This means that it will be accessible to other users of the platform, and potentially even outside the platform. Please do not publish comments or data that you do not wish to share publicly (or do not have the right to share publicly) attached to your app profile (i.e. first name, last name, and profile image).

If there is a propsoed change of ownership of Haven Inc (e.g. merger, acquisition, sale, etc, whether 100% or some smaller part), personal informtion can be transferred to another company in this context.

We might also need to disclose Personal Information in good faith if we need to comply with fulfilling legal obligations under applicable law, regulation, court order, or legal process, or to protect the rights or safety of Haven or other parties, to enforce this agreement (or any other applicable agreement), or responding to legal claims related to the platform.

We may disclose non-personal inforamtion of the platform at our own discretion.

6. Storage and Security of Information

Information used on our platform is delivered through a secure connection using Security Socket Layer (SSL).

Your account is protected by your own personal password, which is fully-encrypted on the server. You are responsible for not sharing this information to unintended third parties as it will give the possiblity of access.

We cannot guarantee the security of your information. We make good faith efforts to protect your information and will keep you updated on any concerns as necessary.

7. Other Non-Haven Services

We are not responsible for other third-parties and other web services. Even though we provide links for user convenince to external sites, we do not claim responsibility for the accuracy or actions of others.

8. Updates and Changes to the Privacy Policy

The effective date of this Privacy Policy is stated at the top of this document. We will notify you of any material changes to this Privacy Policy by giving notice on this document.

As our company grows and evolves, we recommend that you periodically review this page to make sure you have the latest information on our privacy practices.

As this document becomes ammended, the ammended version of this Privacy Policy will supersede all of the previous versions.

If you continue to use the services, you must agree to this privacy policy. If you do not agree to future changes, you agree to stop using the services as of the effective date of these changes.

9. Your decisions

If you do not consent to this privacy policy, you must stop the use of our services. You must decide which information you share with us while using our products and services.

For all things public, do not share information that you would like to remain private.
